Helmet laws in US

I always thought the different states in the USA had two takes on the helmet law thing:
- 'you WILL wear a helmet because WESAYSO you ignorant irresponsible child who can not be trusted to do anything right'
and
- 'whatever blows your hair back man'

Apparently there are other alternatives. Some states make helmets mandatory for people below a certain age. Some states also require you to have your own medical aid before you can go lidless.
