Oberon Clutch Slave Failure - Happy ending!

After 3.5 years and 3210km (a terrible daily average, I know) my Oberon slave did this:

This was about 850km after being removed and refitted for the first time in order to service my clutch oil jet.
Could this have been caused by something I did? I tightened the top & bottom bolts a little at a time to keep it even and didn't tighten them too much. :dontknow:
